CCM for Specialists – Uncommon Conditions

Many specialists act as the primary medical contact for their patients. Patients with complex conditions related to rheumatology, endocrinology, infectious disease, or gastroenterology typically look to their specialists to help coordinate care. They often fear that their primary care doctors or other specialists are not taking into consideration their complex underlying conditions in everyday care decisions. This lack of confidence in doctors who may not be experts in these relatively rare conditions can cause specialists’ clinics to spend a great deal of time with back-office care coordination and other non-face-to-face services.

Chronic Care Management (CCM) is billable under CPT code 99490 and was created as a means to compensate providers who coordinate care. In these circumstances, the most relevant CCM providers are often the sub-specialists. Whether it be a patient with rheumatoid arthritis, HIV, hypogonadism, severe type 1 diabetes, or otherwise, the corresponding specialists can easily offset patient care costs by instituting a CCM program.

Support staff is expensive, and as they spend more time on care coordination, they have less time for other office functions. Your team deserves to be compensated for all the care services it provides – consider capturing these valuable CCM codes now.
